EyeCareConnect Patient Archive Site

The online EyeCareConnect Practice Automation application has been retired. This temporary site is being made available for offices who have not yet downloaded their Patient Demographics (CSV) and Patient Medical History Archives (ZIP) files.

Exports

Patient Demographics List (.csv)
	Preview the first ten records of your patient demographic list in a browser window.
	Export the full contents of your patient demographic list as a comma-delimited file (patients_###.csv). Data is realtime.
Patient Archives (.zip)
	Export your archived patient medical history as an encrypted zip file (arch_###.zip).
	The password needed to open or extract files from Patient Archive zip file (arch_###.zip) after downloading to your computer.
